GRANITE FIELDS INDOOR SOCCER REGISTRATION FORM

All Coaches and adult players are required to sign the Granite Fields Indoor Soccer Registration Form in order to participate in our soccer program. These forms must be completed and turned in before the beginning of your first game.

Teams must be paid in FULL before they will be placed on the schedule.

Please complete the Registration Form and return it with your team payment of $499.00 made payable to Granite Fields. No application will be accepted without the full team payment.

Returned checks are subject to a $25 fee.

All teams and associated spectators are required to follow the Granite Fields rules and regulations. It is the responsibility of the coach to pass this information along to their players and spectators. Failure to abide by Granite Fields rules and regulations may result in expulsion from the facility. ABSOLUTELY NO FIGHTING OR GUM CHEWING IS ALLOWED ANYWHERE ON THE PREMISES!

Granite Fields has a ZERO TOLERANCE rule.
